Checkup:
A Month Later...
Emma reports that her home blood sugar readings have improved slightly but are still high. She admits to a few dietary indiscretions, such as having multiple unhealthy snacks and deserts. For exercise, she has been participating in gym class more but still has not met 30 minutes of sustained exercise per day.
Two Month's Later...
Emma has been improving on getting 30 minutes of exercise per day. Her parents have helped her maintain her blood glucose levels by sending her to school with healthier lunches that include all four food groups.
Three Months Later...
This month Emma has noticed her asthma has improved. She has been able to easily participate in gym class. Her parents also made her a Treatment Plan that they put on their fridge. It concluded of both meal planning and physical activity so Emma could improve.
Four Months Later...
Emma's blood glucose levels have been within normal range the entire month and she has felt very energized and healthier everyday.
Five Months Later...
Emma's parents have noticed by making a schedule monitoring how much television she watches everyday has helped her dramatically. She is now more focused on playing outside with friends. She even has joined a soccer team now that her endurance has improved!
Six Months Later...
At Emma's 6 Month test with Dr Grey she accomplished everything he has asked.
